		If you are a new client and would like to utilize my services, you will need to download each of the following links below. Just click on folder below each title and you will be directed to a form(s) which you will need to print off, fill out, and bring with you to first session. If you are unable to get this done prior to initial session, please come at least 10-15 minutes prior to scheduled time so you'll have time to fill out paperwork Also, please fill out following form below if you are a new client.
