If you are a new client to Daehnert Counseling and would like to utilize our services, you will need to download each of the following links below. Just click on folder below each title and you will be directed to a form(s) which you will need to print off, fill out, and bring with you to first session (or upload them to us in the “New Client Form” button below after you've scanned them into your computer). If you are unable to get this done prior to initial session, please come at least 10-15 minutes prior to scheduled time so you'll have time to fill out paperwork.

Waiting List
If you are trying to set up an appointment with Todd Daehnert and do not see any available times that you need for that week, please
click on the waiting list icon. I will place you on a waiting list on a first-come-first-serve basis and will text or email you when something comes open.
I can usually get you in without a problem within a few days at the latest as I may have a cancellation at a time you desire. Please do not assume you can’t
set up an appointment if you want to see me at a particular time, especially if you were recommended by someone to see me for the first time.
